Policies & Procedures
Ordering Instructions
- Our orthosis fabrication procedures require either plaster casts or FOAMART® Foot Impressions. Ideally a weight bearing dynamic foot imprint should be included.
- For forefoot balancing, please mark all lesions and pressure sites on the cast.
- Send shoes with casts when necessary. Always indicate shoe size and style.
- Mark all plaster casts and FOAMART® Foot Impressions with both doctor's name and patients name.
- Complete the entire prescription form and secure it properly with your order.
- Address the shipping carton completely.
Shipping Instructions
We provide prepaid labels to ship all orders to us. Your orthosis are usually on their way back within 3-5 business days of receipt.
Specialties
Levy & Rappel® is a full service custom orthosis lab. Some of our clients prefer their orthosis to be fabricated using materials or designs that do not appear in the Levy & Rappel® catalog. For information on such items please call us at: (800) 564-5389.
Casts
We automatically store casts for 30 days. Longer upon request.
Guarantees & Adjustments
Ortholen II™ and Carborplast® are guaranteed for one year not to crack or break with normal wear. Forefoot and rearfoot posts on all orthosis carry a six month guarantee. Top Covers on orthosis are guaranteed for 90 days. We will make free adjustments for errors made by Levy & Rappel® within 3 months from shipment date. There is a minimum adjustment fee for any other changes.
Returns & Credits
Levy & Rappel® will not give credit for unclaimed or returned custom orthosis.
Professional Courtesy
The provider is entitled to one free pair of orthosis for personal use after $500.00 of orthosis have been fabricated. The provider's immediate family and staff are eligible for a 20% discount.
Terms
Invoices are sent with each order, and a 5% discount applies if paid within 5 days. Our terms are net 30 days. Interest will be charged at 1 1/2% per month 18% per annum on invoices over 30 days. Prices are subject to change without notice.
Volume Discounts
The discount is applied to charges for custom orthosis and accessories only. Orthoses sent for repairs and adjustments do not count toward earned discounts. All accounts must be current to qualify.

FOAMART® Impression Systems
After careful examination of the foot, mark all areas that will require modifications with an indelible pencil. This will be transferred to the FOAMART® impression to provide Levy & Rappel® with more information. In addition, you may draw a line bisecting the calcaneus so that we will have a reference line called 'Neutral'.
